This case study examines the wound management of a 50-year-old man experiencing homelessness who presented with a severe infected foot wound. Using a person-centred, harm-reduction approach, an interdisciplinary team provided regular wound assessment, infection management, health education and support for housing and nutrition. Following placement in sheltered housing and ongoing community-based care, the client demonstrated improved engagement, reduced pain and complete wound healing within five months. The case highlights the importance of trust-based relationships, low-barrier services, interdisciplinary collaboration and stable housing in improving wound outcomes for individuals experiencing homelessness.
