Summarizing the use of the Wound Prevention and Management Cycle to identify which patients are at risk for skin tears, to prevent skin injuries and to use appropriate skin tear treatment protocols to ensure health-care professionals, working with patients and their care partners, can effectively help patients avoid unnecessary injury, pain and disability.
